Privacy and Authorization

UVA Physicians Group Authorization and Release of Patient Information

In order to protect patient information, the Physicians Group may only release Protected Health Information (PHI) after an Authorization Form is submitted. Please note that this form is required only for obtaining copies of records and does not impact the communication with your care provider in the routine course of patient care.  Patients may use this form to request their own records, or to specify other persons authorized to obtain records on their behalf.

Please note that this form is specific to information maintained in Physicians Group systems.  Release of Information for the UVA Health System may be found on their Medical Records requests page.

Patient Privacy

As a partner in the UVA Health System, the Physicians Group adheres to the same Health System privacy practices. These privacy practices outline the Health System's responsibilities in protecting your financial and health information and confidentiality.
